The caregiver was seen grabbing a child in an inappropriate manner and placing the child into a chair, pushing children toward the restroom, and aggressively placing a child onto a nap mat.

At the time of the inspection there were multiple hazards observed. At the front gate of the facility, that is an entrance to the facility and a gated play area, the latch is broken. This allows the gate to open freely and leaves a risk for children to be able to leave the area. In the older preschool play area a shed has been replaced. Along the side of the new shed there is a gap between the mulch and shed. The shed is stabilized on cinder blocks and there is a gap underneath the shed. This creates a space a child could crawl into, or an animal could easily burrow into. At an exit gate with steps down there is cracked and jagged concrete and a gap created due to erosion of dirt. This creates a gap between the ground and the bottom of the fence and a tripping hazard for children playing. Multiple water hoses were piled next to the building loosely.
